You may apply online here or download the pdf application form and the Statement of Circulation form for business publications (en français) or consumer publications (en français) to send to CARDonline.

There are FOUR steps to this application form:

1. Fill out the general publication information form. Include contacts and publication profile. Submit to CARD
2. Fill out the appropriate circulation information form. Print. Sign. Send to CARD.
3. Send your media kit, including current national rate card, to CARD.
4. Send two recent, consecutive issues to CARD.
